Residency/Required Documents

 

District 19 extends a warm welcome to our new students and families. The following information is presented to help the registration process flow quickly and smoothly. To be eligible for admission to District 19, a child must be 5 years old and less than 21 years of age on or before September 1st of the current school term. A child with exceptional needs who qualifies for special education services is eligible for admission at 3 years of age.

Parents should be prepared to present the following documents and forms when enrolling their child(ren) in District 19 schools for the first time.

Required Documents

1. Birth certificate – certified original copy (hospital copy not accepted).

2. Must present proof of residency within District 19 by providing the required number of documents from each of the following categories:

Category I (One document required)

  • Most recent property tax bill and proof of payment, e.g., canceled check or Form 1098
  • Mortgage papers
  • Signed and dated lease and proof of last month’s payment, e.g., canceled check or receipts
  • Letter from manager and proof of last month’s payment, e.g., canceled check or receipts

Category II (Two documents are required showing proper address)

  • Driver’s license
  • Vehicle registration
  • Voter registration
  • Most recent cable television and/or credit card bill
  • Current public aid card
  • Current homeowners/renters insurance policy and premium payment receipt
  • Most recent gas, electric, and/or water bill

3. Physical examination – required for all new out-of-state students and for students entering pre-kindergarten, kindergarten, sixth and ninth grades. Must be signed by an Illinois physician.

4. Dental examination – required for all new out-of-state students and for students entering kindergarten, second and sixth grade.

5. Proof of immunizations against communicable diseases, as required by the Illinois Department of Health – required within one year before entering pre-kindergarten, kindergarten or first grade, all new students entering District 19, and for students entering fifth, sixth, ninth and twelfth grade.

6. Eye examinations are now required for all new out of state students and students entering kindergarten. Eye exams must be completed by a licensed optometrist or ophthalmologist. In some instances, additional documentation may be required.

Illinois State Law requires children entering Kindergarten, grade 6 and grade 9 have a new complete physical examination AND immunization report performed with a copy of the exam on file with the Health Office before school begins or your student will not be allowed to begin school.

Mid-year transfer students will have 30 days from the date of enrollment to have the following information submitted. All students who are new to Illinois schools also need a complete vision exam.

Preschool

  • 4 DPT (3 Doses by one year of age. 1 additional dose by second birthday)
  • 1 Measles/Mumps/Rubella (1 dose on or after the first birthday, but prior to 24 months of age)
  • 3 Polio (2 doses by one year of age. 1 additional dose by second birthday)
  • Hepatitis B Series (3 doses; third dose must have been given on or after 6 months of age)
  • 3-4 doses HIB (Should be administered at 2, 4, 6 months of age)
  • 3-4 doses Pneumococcal (Should be administered at 2, 4, 6 months of age)
  • 1 Varicella (1 dose on/after the first birthday)
  • Completed physical examination – REQUIRED
  • Lead Screening Assessment (if not completed previously)

Kindergarten

  • 4 DPT (with last one given on or after the 4th birthday)
  • 2 Measles/Mumps/Rubella (with the first one given after 12 months of age)
  • 4 Polio (with last dose given on or after the 4th birthday)
  • 2 Varicella (first dose must have been given on or after 12 months of age)
  • Completed physical examination – REQUIRED FOR FIRST DAY OF ATTENDANCE
  • Lead Screening Assessment (if not completed previously)
  • Vision examination
  • Dental examination

Grade 6

  • 2 Measles/Mumps/Rubella (with the first one given on/after 12 months of age)
  • 3 Polio (with last dose given on/after the 4th birthday)
  • Hepatitis B Series (3 injections at appropriate intervals)
  • 2 Varicella (given on/after 12 months of age)
  • 1 Tdap
  • 1 dose of Meningococcal vaccine for entry to grade 6 or 7
  • Completed physical examination – REQUIRED FOR FIRST DAY OF ATTENDANCE
  • Dental examination

Grade 9

  • 2 Measles/Mumps/Rubella (with the first one given on/after 12 months of age)
  • 3 Polio (with last dose given on/after the 4th birthday)
  • Hepatitis B Series (3 injections at appropriate intervals)
  • 2 Varicella (given on/after 12 months of age)
  • 1 Tdap

Grade 12
